I received a 1099-Q with the box checked that the distribution was taken by a non-beneficiary. The distribution was made to the owner (me) to then reimburse the qualified expenses paid from my personal account to the University. Is there a form you can file with a tax return to show where the expenses were?

It's not needed. Just save the documentation for verification at a future audit.
