There is no form to spread withdrawals over a five year period. Withdrawals are made in accordance with qualified expenses incurred, or according to whatever schedule you set if they're non-qualified.
If you're talking about superfunding an account to spread a deposit in excess of $14,000 over a five-year period, then @dcandmc is right that you would use form 709.
